9. Integratives Management von komplexen Patienten mit hoher Komorbidität

Zusammenfassung

Gängige Muster der Komorbidität bei psychischen Erkrankungen werden beschrieben und gängige Ansätze zur Behandlung komplexer Fälle mit hoher Komorbidität werden überprüft. Depressive Stimmung, Angst, Psychose, bipolare Störung, posttraumatische Belastungsstörung (PTBS) und andere schwerwiegende psychische Gesundheitsprobleme treten oft zusammen (d. h., „sind komorbid“) mit Substanzmissbrauch und medizinischen Problemen auf. Psychische Gesundheitsprobleme sind auch häufig komorbid mit medizinischen Störungen in allen Altersgruppen und können den klinischen Verlauf der medizinischen Krankheit und die Reaktion auf die Behandlung erheblich beeinflussen. Komplexe Präsentationen, die zwei oder mehr schwerwiegende psychische Gesundheitsprobleme und komorbiden Substanzmissbrauch oder ein medizinisches Problem beinhalten, reagieren oft schlecht auf Pharmazeutika und andere westliche medizinische Behandlungen. Die Behandlung komplexer Fälle erfordert eine mehrstufige Behandlungsplanung, die Evidenz aus einer breiten Palette von westlichen medizinischen und CAM-Modalitäten einbezieht. Derzeit gibt es keine Einigung innerhalb der westlichen Medizin oder nichtwestlichen Medizinsysteme darüber, wie man komplexe Fälle mit hoher Komorbidität behandelt. Komplexe Patienten mit schwerwiegenden psychischen Gesundheitsproblemen und komorbidem Substanzmissbrauch oder medizinischen Problemen reagieren besser und schneller, wenn sie von einem Team von Praktikern nach einem kollaborativen Versorgungsmodell behandelt werden. Die in den vorherigen Kapiteln entwickelte Methodik wird erweitert, um komplexe Fälle mit hoher Komorbidität, einschließlich gleichzeitig auftretender psychiatrischer Störungen, Substanzmissbrauchs und medizinischer Probleme, zu behandeln. Wichtige klinische Überlegungen bei der Behandlung komplexer Patienten werden überprüft. Das Ziel der Sparsamkeit besteht darin, den einfachsten Versorgungsplan zu identifizieren, der alle medizinischen, psychischen Gesundheits- und Substanzgebrauchsprobleme ausreichend anspricht. Die Verwendung der Evidenztabellen und Algorithmen zur Planung der integrativen psychischen Gesundheitsversorgung bei hoch komorbiden Fällen wird beschrieben. Das Kapitel schließt mit einem detaillierten Fallbeispiel, das die Begründung für die während der verschiedenen Behandlungsphasen gemachten Empfehlungen enthält.
